• You are at least 66 years old.
• You have an illness or disability (mental or physical).
• You've had this condition for at least 6 months (unless it is terminal)
• You need help or supervision because of it.
• You lived in England, Scotland, or Wales for 2 out of last 3 years.
• You are NOT receiving Disability Living Allowance (DLA), Personal Independence Payment (PIP) or Adult Disability Payment (ADP).
